municipality of Prince Edward Lennox and Addington to access
You will need to have your notice of Assessment
from Revenue Canada (Income Tax form) for both you and your spouse.

Call Prince Edward Lennox and Addington Social Services at the numbers listed above for an appointment and application form. You will need to submit: · a copy of your notice of Assessment provided by Canada Customs and Revenue Agency for both you and your spouse OR Child Care Tax Benefit Notice · verification of accommodations (rent receipt or property tax notice) · verification of employment or school attendance (letter from employer, pay stubs, school schedule or letter from education institution) · court documents or verification of custody where applicable · social medical referral where appropriate
